Registration fees

The registration fee includes:

●  Admission to the Scientific programme
●  Admission to the Commercial Exhibition and Poster Session
●  Admission to the Concert on 4 November 2011
●  Coffee-breaks during the Symposium
●  The Symposium materials

How to register

To make a registration, it is necessary to fill in the on-line Registration Form.

Please note that the on-line Registration Form is only accessible with a password. Simply fill in the Personal Data Form and a password will be sent to you. Your password is the same for all on-line forms.

Please present the Letter of Confirmation at the Registration desk at the Symposium venue to pick up your materials.

Registration forms received after 27 October 2011 will not be accepted as the regular fee. The on-site registration will be available from 28 October 2011.

All participants will receive the Letter of Confirmation within three working days via e-mail or fax after the submission of their registration form.

Visa requirements

Invitation letters for visa purposes can only be sent to people who have completed the registration process. Further information regarding the visa requirements you can find here.

Cancellations and Refunds

The Symposium Secretariat must be notified in writing by fax (+420 284 001 448) or e-mail (praguesymposium@guarant.cz) about the cancellation of the registration. Telephone cancellation will not be accepted. The appropriate refunds will be made after the Symposium.

The following cancellation conditions apply:

Name change

If you should be prevented from attending, you will be given the opportunity to send a colleague in your place. Name changes should be notified in writing to Symposium Secretariat. A handling fee of 20 EUR will be charged for each name change.

Terms of payment

Registration fees must be paid in EUR by Bank transfer or Credit card.

Please note that payment by bank transfer is only possible until 20 October 2011. After this date only payments by credit cards will be accepted (you can either use the online payment system or send us an authorization form as mentioned below).

Bank transfer

The participant's name should be clearly legible otherwise the payment cannot be identified.

All payments made by bank transfer have to be net of all bank charges. The payer pays the bank charges of their bank, and the beneficiary pays the charges of their bank, if any.

To simplify the identification of your payment please enclose a copy of your bank transfer with the registration form and send it to the Symposium Secretariat by fax (+420 284 001 448) or e-mail (praguesymposium@guarant.cz).

Credit cards

For payments through the Online Payment System (available after login) only MasterCard/ Eurocard and Visa credit cards are accepted. These payments will be charged in EUR.

Please note, that payment through the Online Payment System is available once the Letter of Confirmation is received.

In case you would like to use other credit cards such as Diners Club or American Express, please note, that these would be charged in local currency – Czech crowns (CZK) – at the exchange rate of the Czech National Bank valid on the date of payment.

The approximate exchange rate is 1 EUR = 24.557 CZK (September 2011).

For payments using Diners Club or American Express credit cards please complete the Credit Card Payment Authorization Form (applies also to MasterCard/Eurocard and Visa credit card payments) and kindly send it signed by a card holder to Congress Secretariat by fax (+420 284 001 448) or scanned copy via e-mail (praguesymposium@guarant.cz).
